What You'll Do:
Coordinate Comprehensive Care: Seamlessly manage patient care plans, including scheduling appointments, arranging transportation, and facilitating referrals.
Provide Expert Education: Empower parents and caregivers with essential information and advocacy throughout the care process.
Serve as a Central Point of Contact: Connect families with internal and external community services, including screening, diagnostic treatment, and support services.
Address Family Needs: Assist families in resolving financial, psychosocial, and administrative challenges by connecting them with appropriate resources.
Maintain Accurate Records: Develop and maintain systems for documenting, tracking, and monitoring patient data.
Provide Specialized Care: Utilize your pediatric expertise to deliver patient care and resources tailored to specific pediatric conditions, particularly Type 1 Diabetes.
Empower Families: Educate families and healthcare team members on treatment options, community resources, and self-care management.
Foster Collaboration: Collaborate with a multidisciplinary team to address family questions and concerns.
Advocate for Families: Discuss payor criteria and issues with clinical staff and payers to resolve problems.

What We're Looking For:
Bachelor's Degree in Nursing.
Active RN license and eligibility to practice in VA.
3 years of progressive clinical experience, ideally with pediatric patients and Type 1 Diabetes.
Experience with diabetes technology, including CGMs and insulin pumps (preferred).
Basic Life Support (BLS) certification.
Certified Diabetes Care and Education Specialist (CDCES) certification (or willingness to obtain).
Ability to commute or relocate to Fairfax, VA.
